Actually, that was a poor example because taxing one penny has side effects. I would rather save one life and everyone in the world poked with a stick with no other side effects, because I put a substantial probability on lifespans being longer than many might anticipate. So even repeating this six billion times to save everyone's life at the price of 120 years of being repeatedly poked with a stick, would still be a good bargain.
Where there are no special inflection points, a bad repeated action should be a bad individual action, a good repeated action should be a good individual action. Talking about the repeated case changes your intuitions and gets around your scope insensitivity, it doesn't change the normative shape of the problem (IMHO).
