You might want to do your poll as a discussion post, especially if you can find a site that supports a good format.
You can get "check all that apply" polls if you have a paid Live Journal account.
I've made a quick and dirty survey through google forms all responses are anonymous and publicly visible
You need to include "doesn't take supplements".
