People allow doctors to use their best guess in prescribing medicine but don't allow them to retain and share outcome data for off label use of medicine so that their decisions can be evaluated collectively to generate information that might improve future use of medicine. The doctors have to essentially pretend inspired knowledge of what's medically right rather than admitting that they are making a best guess and searching for information with which to later revise that guess.
