I think the problem is mixing heroic responsibility with the idea that responsibility is something you can consistently fulfil. You can fulfil your responsibility as a nurse. Just do your job. Heroic responsibility isn't like that. You will let someone die about 1.8 times per second. Just save as many as you can. And to do that, start with the ones that are easiest to save.
