I recall reading an article about how some Indian hospitals practiced much greater surgical specialization than US hospitals with the positive results you'd expect so I don't think the international situation is quite as hopeless as claimed here. But on the other hand Certificates of Need are a thing in the US that explicitly prevent competition between hospitals.
